Trial Participation Consent
Football is a physical activity and, despite appropriate coaching and safety measures, carries an inherent risk of injury.
By registering for a trial, I confirm that the information provided in this form is accurate to the best of my knowledge and that the player is medically fit to participate, except for any conditions or injuries disclosed above.
For players under 18, I give permission for my child to participate in the Limitless Reach Football Academy trial and associated football activities.
I understand that players must follow the instructions of coaches and academy staff and wear appropriate football clothing, footwear and protective equipment, including shin guards where required.
Emergency Medical Consent
In the event of an injury or medical emergency, academy staff will make reasonable efforts to contact the player’s parent, guardian or emergency contact.
Where urgent treatment is required and the appropriate contact cannot immediately be reached, I authorise Limitless Reach Football Academy to take reasonable steps to obtain appropriate first aid or emergency medical assistance for the player.
Photography & Video Consent
Limitless Reach may take photographs or videos during trials, training sessions, matches and academy activities.
Approved content may be used to showcase academy activities through our website, social media channels and other promotional materials. We will seek to use images responsibly and respectfully.
Privacy
The information submitted through this form will be used for trial administration, player welfare, safeguarding and relevant academy communications.
Medical and safeguarding information will be treated confidentially and only shared with relevant personnel where necessary for the player’s safety and participation.
